Northside Clinic Vic Pty Ltd ACN 134 396 517 as trustee for the Northside Unit Trust has sold the practice to Brecken Fitzroy Medical Centre Pty Ltd ACN 691 642 190. The practice will continue with the same name, clinicians and staff.

What this means for your health record
To ensure continuity of care, custody of your health record will transfer to Brecken on 19th March 2026, not earlier than 21 days from this notice. Brecken will hold your record in accordance with Victorian health privacy law.

Your options
‍If you do nothing, your record will transfer automatically. If you would prefer your record to be sent to another provider, or not transferred, please tell us by 18th March 2026.
